  • Recommendations for future tax planning or if an amendment is advisable. Should your tax professional feel that an amendment is in your best interest, he/she will be able to approximate the cost, as well as the benefits of any actions.

Please include copies of the following documents with your signed Tax Return Review Agreement, as they apply to your situation. (We cannot return originals). You may fax or email or use regular mail.

  1. The tax return to be reviewed.
  2. Form 1040
  3. Form 2106
  4. Form 4562
  5. Schedules/Worksheets/Lists used for expenses, i.e., entertainment, business, auto, etc. applicable to the tax return.
  6. Records used for housing expenses.
  7. Copies of W-2’s and 1099’s.
  8. Pay Package Sheet from the Church if applicable.
  9. Completed Questionnaire regarding other pertinent information.
